TARGETED CASE MANAGEMENT

At Access 2 Improvement LLC. our targeted case managers deliver Mental Health Services to recipients that fall within the guidelines set by medicaid. Per medicaid guidelines, a client must be diagnosed with some form of mental illness in order to receive services from our agency. Upon verification, the client will be assigned a case manager that will initiate an intake , do an overall assessment and develop a service plan.
