Our Medicaid Targeted Case Management (TCM) program helps individuals and families connect with essential medical, behavioral health, and social support services. If you are a provider, hospital discharge planner, or community partner, you can quickly submit a TCM referral online using the form below.

What is Targeted Case Management (TCM)?

 

Targeted Case Management (TCM) is a Medicaid-covered service designed to assist eligible individuals in gaining access to:

  • Medical and mental health care

  • Substance use treatment

  • Housing and food assistance

  • Employment and education resources

  • Transportation and community services

 

Our case managers work directly with clients to coordinate care, reduce hospital readmissions, and improve overall health outcomes.

 

Who Qualifies for TCM Services?

 

Individuals may qualify for TCM services if they:

  • Are enrolled in Medicaid

  • Have a chronic medical or behavioral health condition

  • Need assistance accessing healthcare or community resources

  • Are at risk of hospitalization or institutional placement

We serve children, adults, and families across Florida.

 

How to Submit a TCM Referral

 

Referring a client is quick and secure:

  1. Complete the online referral form

  2. Provide client demographics and Medicaid ID

  3. Select the service needs (housing, medical care, mental health, etc.)

  4. Submit the form

 

A case manager will contact the client within 24–48 hours.
