What is the SMMC Housing Assistance Pilot?

 

“We have partnered with various insurance providers who have adopted an housing assistance program called the housing pilot.”

Access 2 Improvement LLC Authorized by the Agency for Health Care Administration (AHCA), this specific Central Florida program offers targeted intervention during housing crises. “Based on the client's circumstance and criteria, the client may be able to gain access to financial assistance to their housing crisis.” Access 2 Improvement LLC.

 

Who is Eligible for Medicaid Housing Pilot Support?

You may qualify for specialized housing resources through the Sunshine Health Housing Assistance Pilot if you meet these baseline program conditions:

  • Plan Eligibility: You have active Florida Medicaid coverage via a Sunshine Health Managed Care plan.

  • Age Restriction: You are an adult client aged 21 or older.

  • Housing Status: You are currently experiencing literal homelessness or face an immediate risk of eviction.

  • Clinical Diagnosis: You have an active diagnosis of a Serious Mental Illness (SMI), a Substance Use Disorder (SUD), or co-occurring disorders.
